bank refuse to pay policy

I have funeral plan with std bank and my uncle Samson mashiane matsena was one of the beneficiary on my policy who passed on the 2nd June 2015 and claim lodged FNR 15/014660 on the 5th June, I was then told later that they are unable to pay the claim because due to their system error beneficiary fell of the system of which I wasn't aware all the time hence I lodge the claim. I do have 5 funeral plan policy with the bank and I was nor aware as the bank did not notify me. I there request your good self to intervene and investigate the matter or be refunded the past premiums as I did not request cancellation not even sign for.
