After 2 yrs + 9 mnths credit card still not closed

On 14 Nov 2012 I settled my credit card in full. Proof of payment and a request to close it was sent to the Credit Card Div on the same day and my credit card was destroyed. Now, almost 3 years later, I am still struggling to get this account closed. I have phoned them too many times to list here. I have reference numbers and confirmations, I even received 2 letters, one on 29 Oct 14 and an identical one on 5 Nov 14, stating all my details and confirming the above. Unfortunately the credit card number used on these letters is not mine (letters by Vejay Dukhi - Credit Rehabilitation and Recoveries). I wrote an email back pointing this out, but have had no reply.
