Breakfast order - not mushrooms added as requested

I placed a order for 2 Unreal breakfasts for takeaway. I clearly stated that the person I am ordering for is allergic to eggs so please substitute with mushrooms and add a wors as top up with white toast. 2nd order was normal with top up of cheese-griller and brown toast. Firstly I had to speak to 2 staff before I was referred to a manager as neither of them were competent enough to process a simple order. Was put through to a manager and he took my order and said it is fine he will replace the eggs with mushrooms. When I arrived to pay and collect my order it was already packed and I asked the lady if everything is there and she said yes that is my order. I get back to the office only to find that the orders were processed incorrectly. There was no mushrooms as requested and the wors was added to the breakfast with eggs after clearly stating no eggs. Were they expecting us to eat with our hands as they only supp**** 2 knives and no forks. I made more than 20 attempts to call them on their number *** without success as the line was engaged all the time. This is very poor service considering they were not busy and waiters were standing outside the store.
