Re: Payment of service terminations claim

<p>Sir/Madam</p> <p> </p> <p>I, Loyiso Mkongi with ID No. ********** 499082 a biological son of the late Warrant Officer Monwabisi Halifax Mkongi who passed away on the 09 March 2013 working at Bellville Police Station. My late father’s ID.No. ********** 501088, Persal no. ********** .</p> <p> </p> <p>My late father was legally married to my mother Lindiwe Gloria Lamani ID. No. ********** 427083 on the 22 December 1987 and were never divorced. In August 2014 me and my mother were contacted by Mrs Ajauhaar from Bellville Police Station, requesting us to send some documents to enable the Provincial Government to start processing the claim ~ Z894, Z143, Certified Identity documents and child care statement etc were send to Mrs Ajauhaar same month. Year passed by and we did hear any update or progress. In 2015 Mrs Ajauhaar contacted us again to send all the documents that we’ve send the previous year and we did that in September 2015 and again the year passed with no update or progress. In March 2016 a certain Mr Ngwenya from the Provincial Government phoned us also requesting the same documents that we send the previous 2 years, claiming that all the documents that we sent has been rejected since they’ve been there for two years~ all documents sent as requested. When I phoned Bellville Police Station to check the progress, they told me that the claim has been submitted to GEPF for assessment/calculations. Out of desperation i went to the local GEPF at Kwantu Towers here in Port Elizabeth, to my surprise i was told that there was no claim on behalf of my late father submitted.</p> <p>In January 2017 we were contacted by Captain Mashiya from Khayelitsha Police Station in Cape Town requesting us to send all the documents that we’ve been submitting since August 2014,he( Capt. Mashiya) mentioned that the department of Police want to finalise this claim since it’s an old claim. All documents send to Captain Mashiya as requested.</p> <p> </p> <p>In March 2017 Captain Mashiya phoned us telling that he and his team will be coming down to Port Elizabeth as the part of road show for all the old claims. On the 06 March Captain and his team came to P.E.myself, my mother and my sister Ziyanda Cleo Koloi met the team, filled in the forms and submit all the forms to them. This team promised to update us every step of progress but until now we’ve heard anything.</p> <p> </p> <p>All these instances we spent money to send documents via courier service for the department not to update us about the progress and forms end up rejected and we had to re:submit.</p> <p>Kindly assist us with this complaint as i am absolutely do not know where to complain.</p> <p> </p> <p>Regards</p> <p>L.
