On the affidavit please state: ·          Your names ·           Your contact details ·           Your physical address ·         Where and when the bed was purchased ·         Nature of the complaint ·         Pictures of the barcode and label from the bed However, if the mattress was purchased at one of the JD stores (Russels, Morkels, Joshua Doore, Bradlows, Price n Pride, Barnetts, Sleepmasters) the complaint has to be logged with one of their stores as it has to go via their head office
