SASSA contact for original grant papers. Thank you.

Can anyone tell me how to get hold of the SASSA offices. I am trying to get my original application from SASSA to reduce my rates from NMM municipality. I have tried several avenues but I have had no luck. I am severely disabled and cannot leave my home. Please please help me. my original grant form is lost and I need a copy.
