Unauthorised debit order on 15 January

Even though the debit order I initiated last year was only supposed to run until December, I got a shock on the 15 of January when I saw this debit order go off on my account. Firstly, this debit order should not be running anymore, and secondly January is the WORST month for a debit order to go off my account 10 days before I get paid. I also received no warning that this was going to happen.

Please rectify.
