Mandate form not working

I am an Airbnb host. I have been sent the mandate form to fill But it says my email address is invalid, and my phone number is not a valid South African number. I cannot fill in the form and because I can’t fill in the form I won’t be able to receive my funds. This is very bad because I must pay the school fees with that money. Please help me.
