Bidvest Bank's SARB reporting mandate form

Like so many of the reviews here, I'm the victim of Bidvest Bank's SARB reporting mandate form. If you search on Reddit you'll find threads going back many, many months where people have identified the problem as data from the 'first name' field being added to the 'last name' field causing an error message and the withholding of funds. This is exactly the problem for me, where my second initial is being put in the surname as confirmed by service rep Loice. What Loice cannot seem to do is advise what I need to put in the first name and last name field to rectify this issue!!! The fact that they and affiliate AZA Finance have not fixed this issue speaks volumes. I need my payouts and your negligence and poor quality service is not a reasonable excuse to withhold my money!
