Smart shopper card issues

I use to work for a company that took my Smart shopper card and registered it under their name. Now after I left the campany now I'm trying to get a new card under my name with nothing to do with them I can. Tried to contact pick n pay but no luck. All I want them to do is remove me from that companies database so I can get a new card and use it as my own.
