Provident fund

I have a problem I resigned at pick n pay on the 15 of November 2018 I haven't got my provided found money.when I contacted the HR lady at head office she is telling me that she send the documents to Cape town.and she said I must go check at old mutual when I go to old mutual they are telling me that they r still waiting for the claim form and its been 10 months now.any one who can help me how to deal with them?
