Policy Cancellation & Money not paid back!

I cancelled my policy with OnePlan Health Insurance end of November 2015. This was after my first premium had been deducted, but within the 30 day cooling-off period. I was assured that any premiums paid within this time will be refunded. However, it is now 8 January 2016, and I am still waiting! I still have not been sent the form to complete to get the refund!<br> <br> I have called the customer care line and spoken to 3 separate people, who assured me that they escalated things, to God knows who, so that the form is emailed to me. Yet I am still waiting for this form! It is MY money. My policy was cancelled so please give me back MY money!
