Why is my name still on hold under debt review.

Thank you for ruining my life. I received a clearance certificate from you and I got my debt review flag removed from all the credit bureaus. Recently tried applying for a credit card with my private banker to improve my credit score and we encountered issues. I got removed from debt review with my bank and the issue persisted. Until they did a root cause analysis, they called me to let me know that the cause of all the rejection for my credit card application is because Octogen has my name on hold under debt review even though everything is paid up. I even paid for my clearance certificate fees yet I get to have my name withhold under debt review by my debt counsellor for what reason I don’t know because I fulfilled all my payments on time. May you please look into this and have it resolved ASAP as it is blocking my life and there’s absolutely nothing I can do if you keep my name under debt review on your records when it is supposed to be cleared and free. This is not what I signed up for when I chose you guys voluntarily to close off my accounts.
