Prolonged clearance and incompetency

I've been on debt review for over 6 years. In the month of May my debt review was suppose to end, two months later things has still not been finalized. I now have to call creditors to submit paid up letters which I am not making much progress on. I would like receive my clearance letter as I have important things to do that I've been waiting for over 6 years. Now it almost seem like it's impossible as it is taking forever to be finalized.

I've been sending emails and calling Octogen, with no help. All I was told to contact my creditors myself to speed up the process. I am not even sure if Octogen is doing anything on their side as they taking forever to reply to emails. In 2019 I was retrenched had a credit insurance for my debt, but when I called Octogen about it I was told there was no cover but yet deductions has been made. I've asked for documentation and recordings to prove that I did sign up for this spoken to a manager at Octogen, but she never came back to me with any info up to this day. Close to R10 000 has been deducted and no one can account to it. Last month they deducted again even though my accounts have been paid up already. I also have been receiving emails and smses that my accounts has been handed over. So I am wondering if Octogen did indeed pay up my accounts.

I would like to receive answers in regards to the insurance firm Alivio which is an outsource company for Octogen to take accountability for not paying my debts when it was needed and to reimburse me, because the insurance did not stick to their terms and to stop deducting money for no reason. I just want this to be settled so that I can move on with my life and close this chapter with Octogen. I also don't know why I have to pay for a clearance certificate. Where was this stated when I signed up?
