******* Debt review placed under my name by this company Octogen

I was called by one of they consultant who was explaining debt review I asked her a bit of questions and she then advised that she would send me a forms to complete and they would check if they can assist me. a lady by the name of Nokuthula contacted me and wanted to set up a meeting as she needed to collect the original documents from me I then advise her that I am no longer interested this was 2014. and from then I never heard from them a 2 weeks ago I went to do a credit check and they advise me that I am under debt review I was shocked they gave me the number of the company I then contacted the company Octogen and I spoke to a lady by the name of Cheryl Geffen who told me that I never advise them that I no longer what to continue I advise her that I did. and also told her that when the consultant called me she advised that they calls are recorded and now Cheryl is saying my call was never recorded which is very shocking I want this company to remove my name that I am under debt review as this could reflect badly on me. and also companies like this trick people into believing that they are just doing a application and no formal agreement applies while they know very well that they are lying then just want to ***** people money and trap them.
