Have not received requested document after 2 weeks

Good day.<br> I requested a 'paid up' letter on Direct Axis account number *** on the 15th of March 2016 and have still not received it. I tried calling again today but was on hold for 20 minutes before I got disconnected.<br> I need that letter asap
