Kindly Rectify

I have an outstanding loan with Get Bucks and i am currently under debt review (Since 1 February 2015) I received a statement of balance on 19 February for R5232 which i am aware of and busy paying via a distribution agency. But today to my surprise i see on my Credit Profile that i have another loan that was opened on 30 April 2015 with a balance of R5713. I could not have app**** for another loan while under debt review therefore it is clear that there is a mistake somewhere. Kindly advise what is going on because now my debt counselor wants me to pay this account as well.
