Unfaithful dealing on contracts

I have been handed over to creditworx due to unsettled contract according to them. I took a one year contract with Altech Netstar in 2015 september which was due to end up on September 2016. My account was debited monthly up until july 2016 because I cancelled my contract but my account kept on being debited until I stopped the debit. After that I started receiving sms’s around June 2017 that I am owing R748.26 on arrears which I then paid on the 5th july 2017. After that to my surprise I started receiving calls from creditworx alerting me that I am owing an amount of +R5000 to Altech Nestar, immediately I called Altech Nestar and I spoke with Alvina Mashiane who then told me that I had a 3 years contract with Altech which was totally false, I then requested a proof of contract from her and she did promised to forward it, but till today she hasn’t forwarded it and she keeps on ignoring my emails whenever I asked about the feedback.

Furthermore I requested for a detailed breakdown of where that amount of +R5000 is coming from but she failed to do so, and again I sent her the proof of payment for the amount which was on arrears (R748.26), but according to her the amount was accumulated from the 36 months contract which I had with Altech (which I don’t know about).

All I am asking is a proof of this 3 years contract or the telephonical record, as well as the simple detail breakdown of where the amount +R5000 is accrued from?
