urgent complain

03/01/2020 Hospital complaint

Please see the following complaint regarding a J88 request to be completed by the hospital. I went to the hospital to drop off an J88 form to be completed by the doctor that saw my daughter in casualties on the 21/11/2019 . The form was handed to Elize in casualties to get the doctor to complete the form on the 17/12/2019 , I than called in on the 18th of 12/2019 and was advised that she is still awaiting on the doctor to complete the form , I than gave it time up until the Monday , which was the 23/12/2019 and was advised by Priscilla she went on leave . Priscilla than advice me that she will hand it to the doctor to complete , however it is the 3th of Jan and the detective advised me that there he has still not been called to collect the J88 and without the J88 there is no case.
