Patient Neglect at Netcare Waterfall: Lifelong Medication & CIB Form Withheld After Radioactive Iodine

I am posting publicly because Netcare Waterfall management and Dr. Carrim have completely ignored a formal email sent on Wednesday at 10:06 AM regarding severe patient neglect. I was left with dangerous symptoms (facial tingling and near-fainting) with zero support, and my CIB chronic forms are being withheld. Below is the exact email I sent to the Hospital General Manager (Jaco du Preez) and Dr. Carrim, Dr Currim's front lady Shomila and netcare customercare. which has been met with total silence.

I am writing to formally request the immediate release of my complete medical records, recent blood test results, and my fully completed Chronic Illness Benefit (CIB) form so that I can transfer my care to somewhere else.

To date, my experience with your practice and Netcare Waterfall Hospital has been deeply distressing and exhausting. I was admitted to Netcare Waterfall and discharged on 13 May 2026. On that same day, I underwent radioactive iodine treatment to destroy my thyroid, which I was advised would take 3 months to take effect in my body.

Despite being put on lifelong thyroid medication, nothing about my condition, my treatment plan, or my recovery was explained to me by Dr. Carrim. I received my prescription on 18 August 2026, but my blood results were never discussed with me. The prescription was simply forwarded by your front lady, Shomila. I had to repeatedly beg just to receive this script and start my treatment.

Now, I am fighting and struggling just to get my completed CIB form back so that my chronic medication can be registered with my medical aid. This lack of communication and care is completely unacceptable for a patient placed on life-altering medication.

Furthermore, since starting the medication, I have begun experiencing dangerous symptoms: my face tingles at night and I feel like I am about to pass out. Leaving a patient to navigate these severe side effects with zero guidance or communication is putting my life at risk.

Please provide the following within 24 hours so I can seek safe medical help elsewhere: Copies of all my recent blood test results. My complete patient file/history for transfer. The completed and signed CIB Form for my medical aid.

Kindly confirm receipt of this email and let me know when these documents will be emailed to me or ready for collection.
