Not allowed my own medical history

I requested previous medical details and what I got in an envelope is badly copied details that doesnt amount to half my history. From 2014 onwards there is also basically no details?!?!?!

When requesting diagnosis of certain events i was told i am not allowed to get it i must go wherever i was referred afterwards.
