Un-authorised debit order!!!!

On my account there's a debit order (which i definitely did not sign) that's deducted every month! I have reported this and reversed the debit order (which i also have to pay the penalty fees!!!!!).

The details on my account for the debit order is NKB-Rekkunde inskrywingsgelde????
