Still Deducting After ALL DEBIT ORDERS was Canceled

I am in the process of debt review with Nedbank - A year ago, we filled the documents, requesting all debit orders STOPPED. The offer was approved. I am currently the only employed member in the household and 80% of my salary is debited for debt counseling. Today, the small little windfall that I was waiting for to be paid, that I was suppose to use for a little food, was deducted out of my account. The moment it was debited into my account. Now I contributed 2x payments for this month and no food in my house.
