Personal Information sent to unauthorized email addresse

My Nedbank policy insurance information was sent to an email I did not provide to the bank. When approach the branch, they didn't know how it happened as well. Teller contacted group life and they said they got this "unknown" email address from insurance registration. I was lucky they sent email to someone who recognized my surname and forwarded email to me. Please assist
