Nedbank don't want to cancel my credit card

I Mr doodhnath Dhunputh having a problem regarding payment on my credit card. on 17 / 02/ 2014 i had a heartattack. from then I m on tempreaily disability i went Nedbank at phoenix branch regarding payment .A consultant asked me to fill in a disability, critical illness or dread diseases form .which I did fill by my Doctors at I Mr doofhnath Dhunputh having a problem regarding payment on my cridit card. on 17 / 02/ 2014 i had a heartattack. from then I m on tempreaily disability i went Nedbank at phoenix branch fegarding payment .A consultant asked me to fill in a disability, critical illness or dread diseases form .which I did fill by my Doctors at Addington hospital. Submitted forms to Nedbank on 12.13/of December217 from then there is no feed back.consultant adviced me that I have a protection plan.its now being four years I m unemployed .
