"MAGTAPE UNPAID" AND CHARGE UNPAID ITEM I AM ENOUGH I WANT MY MONEY BACK

I am a nedbank account holder and am having this problem each and every month. I have One Debit Order from gym and it deducts R199 every month, when i dont have any fees in my account i receive a debit order saying money has bee debited and around midnight or tomorrow its reflects back again saying "MAGTAPE UNPAID" of the same amount, which means same was reversed and Note - without my authorisation. again R150 get deducted from my account from nedbank even ever i put funds in my account they deduct that R150 even when the gym is not paid, it happend like over 5 times and i want my money back i am fedup with nedbank i want to close my account. PLEASE HELP
