Different account numbers, FICA nightmare and banking charges levied for month before receiving card

I have several queries related to my new Nedbank/American Express account.

Firstly - after an incredible hassle in trying to get my card delivered to me I was advised to go to Tyger Valley branch to collect it. Upon going to the branch I was first told the card wasn't there, and when I refused to leave without my card I was sent to the branch manager who informed me that the card was indeed there but they were told to destroy it as it was supposed to be delivered to my home. She then sent me to Capitec to receive my banking statements for proof of address (FICA). I did this and my card was then FICA'd, my online profile authorised and fingerprinting done.

After receiving my card I registered for online banking.

I then received a call from Nedbank telling me that they'd not received my FICA documentation - and I told them I had done it at the branch. I received another SMS a week later informing me that my card will be deactivated as it was not FICA'd

In addition to this, the account number on my statement and the one listed on my card and online banking are not the same and Capitec won't accept the account number to pay my account.

Also the online banking site states that my cell phone number has not been linked to my account for SMS authorisation.

I therefore:
- can no longer use my card as it apparently wasn't FICA'd - although I was assisted by the Nedbank Tyger Valley branch manager.
- can not use the online banking platform as my cell phone has not been authorised.
- can not pay my account as I have two different account numbers linked to the account - Nedbank: ********** ********** 4, American Express statement: ********** ********** 18
- was levied card/banking fees for the entire month that Nedbank could not locate or deliver my card.

Lastly, I want to query Nedbank's contravention of the requirements for FICA - in fact, the financial intelligence act even addressed this issue on their website stating: "It has come to the Centre’s attention that accountable institutions are applying a restrictive approach in terms of the types of documentation it accepts to verify the residential address of a client. As a result this restrictive approach is frustrating the verification process for clients of accountable institutions."

And yet Nedbank persists in only accepting certain documents for proof of address which are absolutely absurd.

According to FICA - these documents are acceptable proof of address:

a utility bill reflecting the name and residential address of the
person;
a bank statement from another bank reflecting the name and
residential address of the person if the person previously
transacted with a bank registered in terms of the Banks Act;
a recent lease or rental agreement reflecting the name and
residential address of the person;
municipal rates and taxes invoice reflecting the name and
residential address of the person;
mortgage statement from another institution reflecting the name
and residential address of the person;
telephone or cellular account reflecting the name and residential
address of the person;
valid television licence reflecting the name and residential
address of the person;
recent long-term or short-term insurance policy document issued
by an insurance company and reflecting the name and
residential address of the person;
recent motor vehicle license documentation reflecting the name
and residential address of the person.
a statement of account issued by a retail store that reflects the
residential address of the person.

SARS documentation/returns.

utility bill and marital certificate; or
letter from tribal chief or head of residence.

Nedbank, however, only accepts utility, telkom and TV-license accounts as well as car registration (none of which most people have in their name).
