Charged three times for banks error

I get called by the CC division told I am eligible for an upgrade, do I want a Petrol card (Yes), do I want Greenbacks reward (No). I get told I have to take Greenbacks for the first year. Go to my branch to collect CC, they can not find my petrol card but will contact me when they find it. They do not contact me. after some time I go to a branch and request to know where my petrol card is(Destroyed). I get told I will be sent a new card. They charge me for it. I get told the charges will be reversed. They do not get reversed. Yesterday I get a call from the CC division would I like to continue Greenbacks reward I answer NO, this morning I get charged Greenback linkage fee and other fees.
