Change of title and surname

I changed my surname last year at Thavhani Mall, the gentleman who assisted me never changed the title from "Mrs" to "Ms", and then I had the problem of my new card still reflecting my old surname. That got sorted out but am still stuck with the "Mrs" title....I have gone to Florida branch twice already to no avail.

May I humbly request that this gets sorted out.
