Being charged for a credit card I don't have

I did a loan application at Nedbank a while ago to settle my debt. They didn't offer me enough and then offered me a credit card of R1000 which was also useless to me. I didn't supply any proof of Identification nor sign anything or give any payslips or other proof of income. The consultant just did a check on the system on how much I would qualify for. I said I would think about it and return with my documents should I decide to take them up on their offer.

I never heard from them again. I pulled me credit report a few months back and saw that credit card has been listed on my ITC and the balance is like R100. Probably admin fees...... I then sent them a mail explaining that I never even received a credit card. That they can't just open a credit card without my signature or any supporting documents and they should close it immediately as it is effecting my credit score. They rep**** with a fancy email of apology for inconvenience and they would close it. Now I pull my credit record again and its still there with a balance of R168 and apparently I am paying a R25 installment (which I am not).

How does this happen??!!!
