sent a ********** account to debt collection

Good day,<br> <br> i think this is a second time i have to deal with this ********** account that was opened with my partners lost id.. i have reported this sometime in 2011/2012 and thought this was dealt with , however this account has been referred to collections.. please advise what we must do next because we are not doing this again.
