Unauthorised change in security profile

Mweb changed my account security details without my permission. when I called them the callcentre agent could not tell me why as she was unwilling/unable to get that information from the "department that does the changes". She promised to send me the form to correct the login details together with the document that supposedly "authorised" the incorrect details. To date neither has arrived
