Trying to get my name cleared

Below Is the email I wrote to MBD. Advise on the next step to take so I can clear my name.

On the 28th July 2018 , I went to east gate mall in search of a contract Phone, I went to east gate MTN to find out about their deals and one of the customer consultants provided me with information pertaining to cell phone deals they had and requested that I furnish him with my id and payslips to check if I qualified for any of their deals. Unfortunately I did not have my Payslip with me and an agreement was reached that I would return to the store the following day with a payslip so he can finish the process. I never returned to MTN as I had found a better offer at Vodacom and a signed a deal for a P20 (24Months contract) which expires next year in August 2020.

On the 15/08/2018 an account number (A***) was **********ly opened in my name, I do recall going to an MTN branch East gate to enquire about certain deals but not opening an actual account as I highlighted above. Payment went off Sep/Oct/Nov from my account , courtesy of MTN but after I reversed the money I did not get any further deduction. The first thing I did was go to the MTN shop at east gate to get clarity and I was asked to make an affidavit and bring it with me, a week later I returned with it but the MTN consultant could not find me on their system, which made me feel like an idiot actually. This led me to believe It was some ********* trying their luck, because after I reversed the money to my account the account was closed. Nobody from MTN called me regarding this matter and so I let it be. This is now a problem because I can’t even get a loan from the bank due to this. i have emailed MBD and attached documents (affidavits) as advised by one of their consultants and i haven't received any feedback from them apart from sms's informing me that i owe money, this is beyond frustrating and disheartening to say the least, i just want my name cleared.
