REQUEST FOR MARRIAGE CERTIFICATE

<p>I went to a mtn outlet in Cape town to apply for a data sim card. I went with my bank statement, payslip and my identity document. when i had to sign for the application i saw that the application was printed on my ****en surname , I made the consultant aware of that. My application was successeful and i was granted a data sim which i cant use because it is not rica'd iwent twice to notify the branch. i also app**** for additional sims. now im being told that the vetting department wants proof of the different surnames like my marriage certificate my reasoning is that from the start i never app**** with my ****en surname, i even made the consultant aware of the different surnames.</p>
