Poor service with MTN insurance

Am getting frustrated with MTN insurance, they have been billing my account for the insurance premium which I did not sign up for..and I have logged my complaint and I was ask to go to MTN branch to request for the summary of contract and the form that says I am not taking their insurance.. And I have send those supporting docs, and every when I do a follow am not get help...
