Insurance on Handset

I upgraded my contract. Took out insurance on the handset. Was told I need to sign a letterhead from my company for the upgrade. Lost the phone. No insurance was in place. I received no correspondence about them needing a letterhead for the insurance. Who's fault is this now. I'm told by MTN that if there is no proof of insurance on their system they can't expidite the matter further. Surely the onus must be on the store and the consultant who did the upgrade? I have been without a phone from MTN for 4 months now. And I can't cancel the contract. What must I do?

Unfortunately we do not have any records of insurance on your account or any payments made for insurance cover, therefore you handset is not covered with MTN Insurance.
