******* contract added to my "closed" MTN account

Walk into the MTN store and have all my paperwork to sign up for a new contract only to be told by the agent that the account i had back in 2014 which was closed has another contract on, i didnt open this account, which was apparently done this year feb/mar, i now have to go get an affidavit to prove all this when its not a problem on my end. MTN, please fix this mess and deal with the person that opened an account under false pretenses under my name and using my ID.
