I have been paying insurance on a device which no longer has a contract for 4 months

I have been paying insurance on a device which no longer has a contract for 4 months. Everybody blames someone else for not checking to see if I still needed the insurance once the contract ended. And even though I cancelled it in January (after lots of running around ) I cannot get a refund
