I had a debit order from MTN for 3 months for an account I never opened, I called MTN

I had a debit order from MTN for 3 months for an account I never opened, I called MTN they asked me to send them affidavit and close the account which I did on June last year I got a call from debt collectors, I told them that I didn't open the account they asked to do the samething and send them the affidavit which I also did and they never call again, I go to see my credit history I find that I'm in arrays with MTN, I call the dept collectors since I had they numbers and they tell me that the account is closed and refer me to MTN. When I call MTN they direct me to the forensic team and they tell me that they did the investigation and found that the account was opened **********ly and they asked for my email address to me a document to send to the credit bureau it's been three months now and in my credit history shows that I'm in arrays with MTN and I haven't received the documents and calling MTN has been a nightmare if lucky you find someone that can help the team is no way to be found its either they are in a meeting or not there. I don't understand why would someone lie to me and not send a simple thing like an email. Can someone please help me with what I have to do because I sent the same affidavit to MTN and the respond I got from them was;

Good day
