I bought my phone on contract at mall of the North and signed waiver for insurance but they still charged me for insurance

I bought my phone on contract at mall of the North and signed waiver for insurance but they still charged me for insurance. I went to the store to enquire but they referred me to their insurance, I called the insurance that stated that insurance contract was signed in store. I went back to the store several times but they refused that they sent insurance order, I called the insurance whilst in store and the insurance said they got the contract from store on the other side the store is saying they did not send it either, basically I paid for insurance that I didn't sign up for and no one is willing to take accountability when I request for refund.
