********** phone taken under my credentials

Someone **********ly opened a contract phone with Mtn under my names. Provided an affidavit and certified ID as they requested in order for them to release me from being blacklisted. I keep making calls and no one is assisting. I need to apply for a place, I qualify but due to their weak system people were able to do as they wish with my credentials and now i need to suffer.
