Debit for insurance - apparently I have a contract phone with MTN

Pathetic, I've been calling your office your Insurance , I do not have a contract phone with MTN but my account is being debited for an insurance. Your call centre agent Coby/Koby dropped my call without resolving my issue. What is worse I do not know how you managed to get approval to debit from my bank account without my approval
