Contract upgrade that I know nothing about

On the 28th of March I received messages about an order being created and credit check being approved out of nowhere, then I called the customer care line and I was told an upgrade was done at a store but they cannot tell me which store. I want Mtn to investigate what happened and provide me with solutions because I am not going to be blacklisted at the credit bureau due to mtn stores giving new contracts to *****ulent applicants.
