Claim denied and have never received a policy schedule

My phone got lost, when I report to insurance , I was told my claim is rejected because the last used number is not registered in the insurance. Shocking because I am paying cellphone insurance not number insurance. Worst part, I was never informed of such. My policy schedule was never send to me only cellphone contract

I am reporting this to the consumer rights
