Billing for insurance i never requested

Ii have logged numerous calls and even more recently a complaint to resolve the insurance on my contract which i never requested. i have patiently queried this with Insurance department and other channels and not received any response. This has been on going for over 12 Months. Contract was moved from Nashua mobile.
