Adding a contract without my knowledge ***

Could please received attached document of my bank statement and police affidavits statement<br> <br> <br> <br> According to the information I received from MTN consultant my contract is coming to end on 2nd June 2015 and last month you debt my account this amount of R919.19 instead of agree amount of R329.00 so I have come to decision to cut tire with your company by stopping the debt order so that the money that I am owning you, have already debt it on this amount of R919.19 Can please tell me how is that possible that you have add another contractor on my account without my permission which I don't even know.
