*****ulent port out

I was called and offered to port my mrp sim card to another number i gave the number i wanted the porting too happen on, I called days later and the number i had requested the port out to happen on too was confirmed but last night a port out happened to a number i did not request for to happen. This is really *********** how can such a mistake happen? I am so frustrated at this now the number i asked the port to happen on is now not working how is that even possible?
