I have never heard that extracting a tooth with a severe abscess needed pre authorisation, when you are in severe pain this is honestly and truly not an option. This is how a medical aid treat their members even though you are a member for the past 46 years. Don't consider them as a service provider, they are becoming more and more unrealible.
