Bad service

I Would like to launch a formal complain against Moto Health Medical aid Main Member Mr R Vos

I have been battling to get my baby on the medical aid for almost two months now i have send all the documents even clear copies as requested but still moto health has failed to render a service if something happens to my child and he has to go to hospital or gets sick i will keep Motohealth responsible for any cost as Moto Health Medical aid has a lack of customer service i have also nit received any feedback from the medical aid regarding adding my baby boy
