Withdrawal submitted but no work done for a month

I made sure with My HR department that my withdrawal forms for my pension and provident funds were complete correct and submitted on the 2nd of October, I follow up with the agent and Momentum today and the person responsible sends an email to my ex HR person and asks for the forms that were previously sent. This means the business I was to open and start running on the 10th of November will now be on hold for another month. I this is not resolved I will take it further. I need my pay out in order to continue living pay debts and rent as well. This is playing with peoples lives when they have done everything required of them and it is unacceptable. I need this resolved and I need it resolved now. There is also proof of submission by email sent from my HR department.
